Open-Up Assessments Feedback Form

The JSD Modified Unit Assessments are designed for use in PLC groups.  Testing results should be compiled using Mastery Connect. The JSD modified assessments include a variety of question types: open response, multiple choice, and constructed response.  They are designed to assess student understanding of concepts, procedures, and representations.

Follow ethical testing practices when administering assessments, such as those established for summative/end of year assessments.  Provide a testing environment that allows for teacher and student integrity.  Students with IEPs must be accommodated according to their documented goals.  Manipulatives and other math tools such as calculators, multiplication tables, conversion charts, and counters should not be provided unless explicitly stated in the instructions. Keep the assessments secure.

When printing student response sheets, please verify that the images and formatting are clear. If images and formatting are not clear, it is recommended to download the assessment and try printing again.

You must be logged into your JSD Google account in order to access these files. The Optional Pre/Post and JSD Modified Unit Assessments are loaded automatically into your MasteryConnect tracker when you create your tracker from the MasteryConnect Curriculum map.

2024-2025 "Just In Time" Diagnostic Assessments by Standard

  • Administer the Unit Pre-diagnostic Assessment (links are below)  prior to each unit in order to determine scaffolds needed for student learning.  These assessments are found in the Canvas course shared in the JSD Commons.

2024-2025 Open-Up Resources Assessments 

Each Open Up Resources (OUR) Unit has a Pre-Unit diagnostic assessment.  Student performance on the Pre-Unit assessment does not indicate mastery of current 6th-grade content, but rather is an indication of performance on prerequisite skills and concepts.  Teachers can use these items to identify students with particular unfinished learning needs or topics to carefully address during the unit.  Teachers are encouraged to address these skills while working through the on-grade level tasks and concepts of each unit instead of abandoning the current work in favor of material that only addresses previous grade-level skills.  In rare cases performance on Pre-Unit assessments may indicate an opportunity to move more quickly through a topic to optimize instructional time.  Because of the nature of the Pre-Unit assessment and the wide range of grade-level standards covered, Pre-Unit assessments are not included in the 6th-grade MasteryConnect curriculum map.  Performance on the Pre-Unit assessment may be recorded on the Pre-Unit Assessment InventoryYou may also track progress on MasteryConnect and/or the student data template sheet (make a copy).  Student Self Assessment trackers can be found in the Open-Up Resources Drive folder
